Forms


Below is a list of the forms required for the registration process. Please complete all forms and return to a swim team coordinator or the Sonoma Ranch Clubhouse.
 
Cover Page

Please add this page to the front of the packet and staple together.  It will help us with the registration process.  The packet will include at least one form each listed below, with the possiblity of multiple registration pages (one per swimmer).

Registration and Waiver Form

Required for all swimmers; If all of your information has remained the same as last year you may write “SAME” but please complete the t-shirt information at the bottom of the form because the kids grow. 
Correct e-mail information is VERY important on the registration form.  Please verify we have a cell phone number where you can be reached at all times.  The waiver form is required for insurance purposes and we only need ONE per family.  All registered swimmers will receive a free t-shirt.  If additional t-shirts need to be ordered, please fill out the bottom portion of the registration form so we will NOT miss the extra orders.


Meet Availability

This will be posted as soon as the meet schedule is approved.  One required for each family.
This form greatly assists coaches and team reps in the time consuming and complicated strategy of seeding swimmers into events, especially in regard to the relays. Relays are perhaps the most fun aspect of a meet however, if one swimmer does not show up, the relay team does not swim. It is very disappointing to the other swimmers if a team member does not show up.
